Cheekbone surgery, also known as malar osteotomy, is one of the most popular procedures that experienced doctors perform to improve the face of patients whose cheekbones give it a square look, which can lead people to lose confidence.

Many patients have cheekbone structures that require a reduction to their malar projection to achieve an oval or heart-shaped face. Cheekbone Reduction is a surgical procedure that reshapes prominent cheekbones to help patients get more beautiful and balanced facial features. Additional surgery can also be performed to correct droopy cheeks and restore a youthful appearance.

Who is a good candidate for Cheek Reduction Surgery?

Patients with prominent, rounded cheeks or those who have a narrow face and wide cheekbones. The ideal candidate must also be healthy and have realistic expectations. 

Cheekbone Reduction surgery is not recommended for patients under the age of 18 or those with unrealistic expectations. This procedure will not improve the facial contour of patients with large cheeks. 

In general, a good candidate for Cheek Reduction Surgery is:

  • A patient who is physically healthy
  • A patient with stable weight
  • Someone with realistic expectations and a positive outlook
  • Non-smoker
  • A patient at an age where facial growth is complete
  • A patient with too high cheekbones

Questions To Ask A Facial Plastic Surgeon Before Cheekbone Reduction Surgery

An experienced surgeon will take care to answer every question that a patient may pose before and after performing any procedures. It is often advised for a patient to list the questions on paper in advance of a cheekbone reduction consultation.

Some key questions to ask a primary surgeon before any plastic surgery include:

  • Are you a board-certified plastic surgeon?
  • Do you have specific experiences with other facial plastic surgeries, such as chin augmentation, hair transplant, or rhinoplasty? If not, do you have experience performing cheek surgery and other related procedures?
  • Do you have extensive experience in cosmetic surgery, reconstructive surgery or craniofacial surgery?
  • Are the results I am seeking realistic?
  • What are the limitations of cheekbone reduction surgery?
  • Will there be visible scarring?
  • Would I need pain medication during my recovery period?
  • What are my treatment options?
  • Are there any concerns that I should be aware of?

  • Will I be given local anesthesia or IV sedation?

Preparation for Cheekbone Reduction

In preparation for Cheek Surgery, a patient may be required to:

  • Schedule a consultation with a plastic surgeon
  • Complete an ECG (for patients over 40)
  • Complete a physical examination and lab tests, including a blood test and chest X-ray
  • Stop taking Aspirin, anti-inflammatory drugs, and herbal supplements at least two weeks prior to the procedure
  • Stop smoking at least two weeks before and after the surgery
  • Stop drinking alcohol at least two days prior
  • Get a facial X-ray

Procedure for Cheekbone Reduction Surgery

Transgender Thailand’s partner hospitals use a L-shape osteotomy or two-site osteotomy and rotation method to reduce a patient’s cheekbone. Surgeons using this procedure create two-incision scars, one of which is done inside the person’s oral cavity and the other in front of the ear behind the sideburn.

After making the cut, the surgeon will rotate the cheekbone inwardly and fix it in place using a mini-titanium plate and screws. The intraoral incision will be closed using absorbable sutures, while the external incision is closed using fine sutures. 

On average, Cheekbone Reduction surgeries take two hours to complete under general anesthesia. Patients are recommended to spend two nights in the hospital, under the nurses surveillance. 

Post-operative care

Patients who undergo Cheekbone Reduction will be required to frequently use mouthwash to prevent any infection. Additionally, patients should also keep their faces elevated to help reduce swelling following the surgery. 

Other postoperative care guidelines may include:

  • Avoid strenuous activities for a month
  • Stop smoking or drinking alcohol for at least two weeks
  • Attend all follow-up appointments
  • Remove stitches a week after cheek reduction surgical procedure

Patients may return to work 10 days after the surgery, though there may still be some swelling. 

It is also important to take the medication prescribed by surgeons to speed up the healing process and prevent infection following cheekbone reduction surgery.

*The average recovery time for this type of plastic surgery ranges between two to seven days.

Risks and complications

Cheekbone Reduction is a low-risk procedure. However, like many surgical procedures, there are certain complications associated with it, including:

  • Numbness
  • Changes in skin sensation over the surgical area
  • Asymmetry 
  • Tightness when opening the mouth
  • Zygomatic arch fracture
  • Unstable zygomatic bone
  • Zygomatic bone and arch malposition

Patients who develop concerning symptoms, such as excessive bleeding or infection are advised to seek medical services immediately.

How Much Does Cheekbone Reduction Cost?
