Eyelid Surgery Before and After Procedures and Post-Op Care

Eyelid surgery, also called Blepharoplasty, is a cosmetic procedure designed to improve the appearance of a patient’s eyelids. This surgery gives individuals an upper eyelid crease, which can make a person appear more youthful.  Transgender Thailand’s partner hospitals offer a number of Blepharoplasty techniques to adjust the size and shape of a person’s eyelids.

Double Eyelid Surgery

In general, many Asians have little to no eyelid crease. This surgical procedure is quite popular among those who have smaller eyes, single eyelids, or unclear double eyelids. There are two techniques commonly used during this procedure:

Eyelid Surgery
  • Open technique. The surgeon makes an incision into the patient’s eyelid crease. Through this incision, the surgeon can access and remove loose fat and excess skin. The surgeon can now meet the patient’s desired aesthetics by stitching the skin close to create a new eyelid.

  • Suturing technique The surgeon marks six points on the new eyelid crease. These six points will determine where the surgeon will insert the suture to create the new eyelid.

Eye ptosis correction is a surgical procedure used to correct drooping eyelids. There are many techniques used during this procedure, which depend on the severity of the drooping eyelid.

For mild eye ptosis, the surgeon can fixate a Levator application to correct the drooping eyelid.  For severe ptosis, the surgeon will use the frontalis muscle flap from the patient’s forehead and fixate a tarsal plate to make the eye more open.

Lower Eyelid Surgery (Lower Blepharoplasty)

This type of surgery is used to correct the excess skin, excess fat, and wrinkles of the lower eyelid. This gives the patient a smoother under-eye, which adds to their youthful appearance.

  • Transconjunctival Lower Eyelid Surgery
    This is suitable for a patient who has eye bags but still has good skin elasticity. The surgeon will remove the excess fat through an incision in the conjunctiva and close the incision with dissolvable stitches.

  • Transcutaneous Lower Eyelid Surgery
    This technique is typically performed on patients with loose skin, bags, or wrinkles due to a lack of skin elasticity. The surgeon will make an incision just below the lower eyelash. This incision will allow the surgeon to access and remove excess fat under the skin.

Epicanthoplasty

This cosmetic procedure reduces the patient’s “Mongolian fold,” which is the skin of the upper eyelids that covers the inner corner of the eyes. During this procedure, the surgeon would make a small incision on the inner corner of the eyes to improve its shape.

  • W-Epicanthoplasty
    The surgeon will make a W-shaped line at the inner corner of the eyes. This method is recommended for people with a severe Mongolian fold.

  • Z-Epicanthoplasty
    This method is used to change the direction of the inner corner. This procedure does not require skin removal.

  • V-Epicanthoplasty
    This procedure keeps the top of a person’s medial canthus pointed slightly downward and is typically performed on people with a mild Mongolian fold.

Canthoplasty

This is a surgical procedure that removes a person’s lateral Canthus to give their eyes a more balanced look.

  • Lateral Canthoplasty
    This technique is used to create a change in the horizontal dimension of a person’s eye. It can enlarge the eye’s outer edge by approximately three millimeters.

  • Canthopexy (Cat eyes)
    This surgical procedure strengthens and stabilizes the lateral canthal tendon, repositions the outside corner of the eye, and restores a person’s youthful appearance.

  • Lowering Canthoplasty
    This procedure can move the Cathus downward by up to five millimeters, significantly improving the balance and shape of a person’s eye. 

Questions To Ask Plastic Surgeons Before Getting Eyelid Surgery in Thailand

Dedicated plastic surgeons will take care to answer every question that a patient may pose before and after performing plastic surgery. It is often advised for a patient to list the questions on a paper in advance of the consultation.
Some key questions to ask before getting eyelid surgery in Thailand include:

  • Are you a board-certified plastic surgeon?
  • Will I benefit from getting a lower eyelid surgery/double eyelid surgery?
  • Are my expectations for upper and lower eyelids surgery realistic?
  • Will I be given general anesthesia or local anesthesia during the procedure?
  • Does the procedure conform to international standards?
  • Will I need to stop drinking alcoholic beverages prior to the cosmetic surgery?
  • When do I need to return to the Bangkok plastic surgery clinic following the procedure?
  • In case of emergencies, should I go straight to the cosmetic surgery centre?

Postoperative Care

  • Apply a cold pack on the surgical site for four hours following the surgery. This helps reduce swelling
  • Sleep with the face slightly elevated for a week following the surgery
  • Take extra caution when coughing or sneezing to prevent bleeding
  • Avoid smoking or drinking alcohol for two weeks after the surgery
  • Avoid eating salty, spicy, or preserved food for two weeks

  • Swelling
  • Bleeding from the incision line
  • Dryness to the eyes
  • Sensitivity to the sun or other bright light
  • Difficulty closing the eyes
  • Infection
  • Pain
  • Visible scarring
  • A temporary or permanent change in vision
