Breast augmentation, or augmentation mammoplasty, is a staple plastic surgery that has been utilised by women who want to expand their cup size. However, while it is a common surgery, it is more complex and complicated to perform breast feminization surgery. The reason is that the anatomy is different and it requires an expert plastic surgeon to ensure quality results.

There are physical differences to consider when preparing for MTF Breast Augmentation. Firstly, a transgender candidate tends to have a chest that is wider. The candidate also often has a smaller nipple shape and areolar, which could be positioned differently than those seen on the breasts of a female candidate.

Because of these complexities, a plastic surgeon must helm breast augmentation procedures as the result of these surgeries. Often, patients who are unhappy with these subpar results seek a team of specialists to make their breast shape look more natural.

Prior to the surgery, breast augmentation patients must have been on Estrogen hormone therapy for a minimum of one year as this helps mimic breast mass, breast fat, and breast tissue, which are important in the breasts’ growth. However, it’s very rare for hormone therapy to make breasts grow into the size and shape that most patients desire.

Incisions for Breast Augmentation

The use of cutting-edge techniques partnered with surgeon expertise is necessary to help ensure that breasts balance perfectly on both sides and that nipples and areolar match too.
When doing breast augmentation surgery there are three locations where plastic surgeons could make the incision:

  1. Periareolar incision – This incision passes through the milk duct and is made at the junction of where the pigmented areolar skin meets the natural breast skin. The incision is approximately 1/3 of the areolar in size. The advantage of this is that due to the incision location, the color of the pigmented skin can help hide it.
  2. Transaxillary incision – This incision is made under the armpit and is around 4-5cm in size. It is made in the armpit’s natural crease, where a submuscular pocket is created in which to insert the breast implant. However, this incision may lead to the implant being a little high due to the incision area. The scar will be visible only when patients raise their arms. There is a limit of 250/300 maximum cc. that can be inserted via this type of incision.
  3. Inframammary incision – This is the most commonly used incision and is approximately 5-6cm long and created at the base of the breast. Because of the incision location, this allows for direct access to the submuscular/subglandular plane without going through the milk ducts. However, it will leave a visible scar.

The incision location depends on many factors including, but not limited to, the breast size that a patient wants, the patient’s existing anatomy, the type of implants they will be receiving, and the results of the surgeon’s physical examination. Surgeons always try to choose the option that will leave less scarring.

Where the surgeon places breast implants is another consideration in MTF breast augmentation surgeries. There are three places to choose from:

  • Putting the breast implant under the breast mass
  • Putting the breast implant under the muscle of the chest wall
  • Putting the breast implant on the Dual Plane, which is a combination of both

Who are good candidates for Male to Female Breast Augmentation

In order to be considered for this surgery, a patient must fulfil important criteria:

  • For a better surgery result, patients are advised to take the estrogen hormone for at least one year before the Breast Augmentation surgery.
  • Patients must be in overall good health
  • Patients must have realistic post-operative expectations
  • Patients must be 18 years of age and above
  • Non-smokers
  • A patient who is not satisfied with the size and appearance of their breasts following significant weight loss
  • A patient with drooping breasts

Preparing for Male to Female Breast Augmentation Surgery

Before the surgery, there are steps all patients need to take. These steps include:

  • Proper consultation with the plastic surgeon to select the best option for breast augmentation based on the patient’s goals, budget, and physical condition.
  • A full physical examination to make sure there are no health concerns that may impact the success of the surgery.
  • Stopping any medications that will impact blood clotting. These can include aspirin, ibuprofen, and herbal supplements. These must be stopped a minimum of two weeks pre-surgery.
  • In some cases, patients may be encouraged to prepare for a one night stay following the surgical procedure.

Questions to ask your surgeon before Breast Augmentation Surgery in Bangkok, Thailand

Dedicated plastic surgeons will take care to answer every question that a patient may pose before they perform breast augmentation cosmetic plastic surgery. It is often advised for a patient to list the questions on a paper in advance of the consultation.

Some key questions to ask a plastic surgeon before Breast Augmentation surgery include:

  • Are you a board-certified plastic surgeon?
  • How many years of experience do you have in cosmetic surgery?
  • Do you have specific experiences with reconstructive surgery, such as breast enhancement or breast implant surgery?
  • Are the results I am seeking realistic?
  • What are the limitations of breast implant surgery?
  • Will there be visible scarring?
  • Would I need pain medication during my recovery period?
  • What type of breast implants would be most appropriate to achieve my desired outcome?
  • What are my treatment options?
  • Are there any concerns that I should be aware of?
  • How much does breast surgery cost?
  • Would I need revision surgery?
  • What is the treatment plan and follow-up care plan?

How is the MTF Breast Augmentation Procedure Done?

Breast augmentation surgery is done using general anesthesia. Once the patient is under, the surgeon will make the incision and the pocket for the implant. They will also adjust breasts to help create the best shape.

Once the implant is placed and shaping, the incision is then closed using layered stitches.

In some cases, the surgeon may place small drainage tubes in the pocket created in the chest area. These tubes help drain accumulated fluid following the plastic surgery.

In total, the procedure usually runs from 1 up to 2 hours.

After the MTF Breast Augmentation procedure, the breast area may be painful for several days, but this can be mitigated with painkillers. There may be some discomfort and soreness as muscles recover. This will improve significantly within a week from surgery.
Normal exercise may be resumed roughly two months after the surgery. Breasts should be carefully massaged daily to prevent the scar tissue from hardening (capsular contracture).

Breast Augmentation Post-Operative Care

These are the important reminders for post-operative care for breast implant surgery:

  • Keep the incision wound clean and be sure not to get it wet for at least 3 days after the procedure.
  • The stitches will be removed 5 to 10 days after the surgery, depending on the incision site.
  • Be sure to keep all appointments with the surgeon for follow-up.
  • Do a breast massage after your stitches are removed, once a day, every day, for a minimum of 3 months or longer, to prevent hardened scar tissue.
  • Use compression binding bras for roughly 2 months following the procedure.
  • Do not lift or carry heavy items (2-3kg), at least for 2 months to prevent implant movement.

Following breast lift plastic surgery, patients may also be advised to wear a wireless sports bra or compression bra to reduce healing time, swelling and pain. Following the doctor’s orders will also ensure that the surrounding tissues heal properly, which will result in the patient’s desired outcome and avoid complications.

Potential Complications

Breast Augmentation is a common surgery and risks are very rare. But some of the potentially rare risks include:

  • Hematoma
  • Bleeding
  • Swelling
  • Infection
  • Asymmetry
  •  Pain
  • Poor Healing
  • Scarring (that may sometimes be thick, sore, or red)
  • Hardened or contracted scar tissue that may cause distortion in shape or pain
  • Implant failure
  • Decreases sensation (usually temporary)
  • Risks associated with anesthesia
